Next, move to the 'EXTERNAL INSTITUTION INFORMATION' section. Provide the name of the external institution you plan to attend, its campus location (city and state), and the term of attendance.
In the 'External Course' section, list each course you intend to take. Include the course number and title for each course along with its corresponding credits.
Ensure that you have your Assistant Dean's signature for approval on each course listed. Additionally, indicate if the courses are already in the matrix by selecting Yes/No options.
Finally, review all entries for accuracy before signing and dating the form. Once completed, return it to the Registrar's Office as instructed.
