Electronic scope of appointment 2026

1. On the Home Page, select the green Scope of Appointment button. This will open the electronic Scope of Appointment form.
2. Complete all required fields on the first page of the form. This includes selecting the plan types you wish to discuss and entering the consumer’s demographic information.
3. Proceed to the signature page where the consumer must sign the electronic SOA. They can do this using a standard touch/mouse signature or by utilizing Remote Signature.
4. After signing, you will receive a confirmation number beginning with an 'S' for your SOA. You can view your completed SOAs in My Applications under the My SOA Confirmations tab.
