2. Begin with SECTION A: SITE CHARACTERISTICS. Enter the date of visit, grantee number, and partner ID if applicable. Fill in the CLIENT RHHT ID and GPRA ID numbers as required.
3. Proceed to SECTION B: DEMOGRAPHICS. Select your gender, ethnicity, and race by checking the appropriate boxes. Provide your age and indicate if you have had a previous HIV or viral hepatitis test.
4. In SECTION C: RISK BEHAVIORS, check all relevant boxes regarding your sexual activity and substance use over the past 30 days.
5. Move on to SECTION D: Rapid HIV TESTING RESULTS. Indicate your rapid HIV test result and whether you received this result.
6. If applicable, complete SECTION E for Rapid HEPATITIS B & C TESTING RESULTS by checking the results received for both Hepatitis B and C tests.
7. Continue through SECTIONS F, G, H, and I to provide confirmatory testing results and types of services provided as necessary.
