2. Begin by entering your Student Information. Fill in your SJSU ID number or the last four digits of your SSN if you do not know your ID. Include the years you attended, degree received (BA or BS), and your personal details such as first name, middle initial, last name, email address, and phone number.
3. In the Name Printed on the Diploma section, provide your legal name as it should appear on the diploma. If you wish to use a different name, list it in the designated fields for first name, middle name, and last name.
4. Next, complete the Mailing Information section by entering your address, city, state/region, postal code, and country if applicable.
5. For Payment Information, indicate how you will pay. If paying by check or cash at the cashier's office, ensure to submit this form there. Specify the quantity ordered and sign at the bottom certifying that all information is accurate.
