  2. Begin by entering your name in the designated field at the top of the form.
  3. Select the account type you wish to pay from by checking either 'Visa' or 'MasterCard'.
  4. Fill in your account number or account ID. Remember, a separate form is required for each account.
  5. Choose one deposit account to debit: Checking, Savings, or Money Market. Fill in the corresponding account number.
  6. Attach a voided check or deposit slip from your selected account as instructed.
  7. Indicate your desired payment option for monthly payments by checking the appropriate box and filling in any necessary amounts.
  8. Specify the start date for automatic payments, ensuring it is at least 30 days in the future.
  9. Sign and date the form. If applicable, have a joint cardholder sign as well.

If supported by your payment processor, you can initiate an authorization reversal to expedite the release of funds. If not supported, you may contact the cardholders issuing bank to request the reversal.
To revoke a live mandate: On your mobile device, open the Google Pay app . At the top right, tap your Profile picture. Select Autopay. In the Live tab, tap the mandate you want to change. Select an option: Enter your UPI PIN. To confirm that the mandate is paused or canceled, check the status in the Live tab.
To confirm a mandate request, you need your UPI PIN. You dont need to enter your UPI PIN to decline a mandate request. You can also approve an Autopay mandate through an in-app request that you receive on Google Pay. Tip: Autopay requests may take up to 24 hours to reflect on the Google Pay app.
