1. Click ‘Get Form’ to open the Skateboarding Competition entry form in the editor.
2. Begin by filling out the Competitor Information section. Enter your full name, address, phone number, age, and email. If you are under 18, ensure to include your parent's contact information.
3. Select your division from the options provided: Beginner, Intermediate, Advanced, or Senior (30+). This helps categorize participants based on skill level.
4. Complete the Emergency Contact Information section by providing a full name, primary and alternate phone numbers, and their relationship to you.
5. Read through the waiver carefully. If you are over 18, sign and date it. If under 18, have a parent or guardian sign the waiver as well.
6. Once all fields are filled out accurately, save your document. You can then email it to info@skateeastwood.com or bring it to onsite registration.
