Qc form 2025

1. Click ‘Get Form’ to open the qc form in the editor.
2. Begin by entering your NAME & HULL # at the top of the form, followed by the CONTRACT/TASK ORDER/CLIN/TWD and LOCATION OR TANK details.
3. Fill in the WORK ITEM and REQ’T DOCUMENT fields, ensuring you also specify the fiscal year (FY), TABLE, LINE, and COLUMN as required.
4. In the QA Equipment No. section, refer to Appendix 2 for accurate equipment identification.
5. For each Area/Location/Edge (A, B, C), input your readings in Reading (1), Reading (2), and Reading (3) fields. Calculate and enter the average for each set of readings.
6. Indicate whether each area is Sat or Unsat based on your findings. Ensure all averages are calculated correctly for WFT/DFT.
7. Complete the remarks section if additional notes are necessary. Don’t forget to sign and date at the bottom of the form as a certification of accuracy.
