The Individual Family Service Plan (IFSP) is a document that is used to provide early intervention services for your child (birth to age three). It contains information about your childs five areas of development: cognitive, physical, communication, social and emotional, and adaptive skills.
The Individualized Family Service Plan (IFSP) is both an agreement and a legal document that puts in writing information about your childs development, your familys priorities, your childs strengths and needs, and the outcomes you and the IFSP team would like to have your child achieve.
The plan states the familys desired outcomes for their child and themselves and lists the early intervention services and supports that will help meet those outcomes. It also describes when, where, and how the services will be delivered. The IFSP is developed by a team of key members.
The plan should include a personalized statement of the persons expectations for the future and state who will be responsible for providing the supports and services to docHub those goals.
