In Section 1, the agency should fill out the Department, Division, Employee ID, Name, Classification Title, Class Code, Grade/Step, Seniority Credits, Date of Notice, and Reason for Termination. Ensure all fields are accurately completed.
For reemployment rights, indicate 'YES' or 'NO' and specify the Layoff Date. If there are no reemployment rights, provide an explanation.
The authorized agency representative must sign and date the form at the bottom of Section 1.
In Section 2, the employee reviews the information provided by the agency. They must confirm their agreement by signing and dating this section.
Finally, ensure that any additional calculations regarding seniority credits are completed in Section 3 for personnel use only.
