How to use or fill out emdeon epayment with our platform
- Click ‘Get Form’ to open the Emdeon ePayment Enrollment and Authorization Form in the editor.
- Begin by completing the 'Bank Account Information' section. Enter your financial institution's name, account owner name, and account type (checking or savings). Ensure you provide accurate routing and account numbers.
- Fill out the 'Corporate or Organization Level' section with your full legal name, provider email address, and contact information. This ensures that all communications are directed to the correct entity.
- In 'Payer Specific Information Form', list all payers you wish to enroll for electronic funds transactions. Make sure to include their respective payer IDs and contact numbers for any necessary follow-up.
- Designate authorized representatives in the 'Authorized Representatives Information' section. This allows specific individuals to manage your enrollment and transaction processes.
