5. Provide details about the event including its title and location. Indicate whether you have attended this event before and provide the date if applicable.
6. Complete the inclusive dates of the event and registration cut-off date. Include sponsor information and whether it qualifies for continuing education units or credits.
7. Estimate costs associated with participation, including course fees, vehicle expenses, lodging, food, and miscellaneous expenses. Ensure to calculate total estimated costs accurately.
8. Sign and date at the bottom of the form as both requesting staff member and approving supervisor after filling out all necessary fields.
