- Begin with Section I, where you will enter the Sponsor Information. Fill in the Sponsor's Social Security Number or DoD Benefits Number, full name, and date of birth.
- Proceed to Section II for Individual Enrollments. Here, provide your mailing address, residence address (if different), and contact numbers. Specify your preferred Military Treatment Facility (MTF) and provider's name if known.
- List any enrolling family members by entering their names, dates of birth, addresses, and contact information. Indicate their preferred MTFs as well.
- In Section III, read the acknowledgment statements regarding TRICARE Plus. Sign and date the application to confirm that all information is accurate.
- Finally, submit your completed application to the designated Military Treatment Facility and keep a copy for your records.
