2. Begin with Section I, where you will enter the Sponsor Information. Fill in the Sponsor's Social Security Number or DoD Benefits Number, full name, and date of birth.
3. Proceed to Section II for Individual Enrollments. Here, provide your mailing address, residence address (if different), and contact numbers. Specify your preferred Military Treatment Facility (MTF) and provider's name if known.
