Purpose. Use Form 1024: to document the items or services that result in an Individual Service Plan (ISP) or Individual Plan of Care (IPC) exceeding the assigned cost limit; to establish the medical need and rationale for these items or services; and.
Below is a list of programs: STAR is a program for children, newborns, pregnant women and some families. STAR+PLUS is a program for adults with a disability, people 65 or older, and women with or cervical cancer. STAR Kids is a program for children and adults 20 and younger with a disability.
