1. Click ‘Get Form’ to open the DE 9C in the editor.
2. Begin by entering your employer account number and the quarter ended date at the top of the form.
3. In Item A, input the number of employees who worked during the payroll period that includes the 12th of the month.
4. If applicable, check Item B for Voluntary Plan Disability Insurance wages. Otherwise, leave it blank.
5. For each employee, fill in their Social Security Number (Item D), full name (Item E), total subject wages (Item F), PIT wages (Item G), and PIT withheld (Item H).
6. At the end of each page, calculate and enter totals for subject wages (Item I), PIT wages (Item J), and PIT withheld (Item K).
7. On the last page, provide grand totals for all items as required in Items L, M, and N.
8. Finally, sign and date Item O to certify that all information is accurate.
