1. Click ‘Get Form’ to open the RT-6 form in the editor.
2. Begin by entering the quarter ending date in the designated field. Ensure you use black ink for clarity.
3. Fill in your RT Account Number and F.E.I. Number as pre-printed on the form. If you do not have an account number, follow the instructions to register.
4. In Section 1, enter the total number of full-time and part-time workers who performed services during the payroll period.
5. Complete Sections 2 through 9 by providing gross wages, excess wages, taxable wages, tax due, penalties, and interest as applicable. Refer to instructions for calculations.
6. Sign and date the form at the bottom. If applicable, include preparer's information if someone else is completing it for you.
