2. In Section A, begin by selecting your choice of superannuation fund. Place an 'X' in the appropriate box for either an APRA fund, a self-managed super fund (SMSF), or the employer's nominated fund.
3. Complete your personal details including your name, employee identification number (if applicable), and tax file number (TFN). Providing your TFN is optional but recommended to avoid higher tax rates on contributions.
4. If you chose an APRA fund, fill in the required details such as Fund ABN, Fund name, and contact information. Ensure you attach a letter from your fund confirming it will accept contributions.
5. For SMSF nominations, provide similar details as above and confirm that your SMSF will accept contributions by placing an 'X' in the designated box. Attach necessary documentation as specified.
6. Finally, sign and date the form in Section A before returning it to your employer promptly.
