The FORM DS1500 is a document completed by healthcare professionals to enable terminally ill patients to claim Disability Living Allowance (DLA) or Attendance Allowance (AA) under 'Special Rules' if death is expected within six months. It is not a claim form itself, and must accompany an appropriate claim form. Patients qualifying for the terminal illness test receive higher benefit rates, which may be backdated.
