2. In the first section, enter the federal employer identification number, business name, and address of the contractor awarded. Select either 'Government Entity' or 'Prime Contractor' as applicable.
3. Proceed to the second section and input the federal employer identification number, business name, and address for the contract awarded to. Choose between 'Prime Contractor' or 'Subcontractor'.
4. Enter the Government Issued Purchase Order Number in the designated field.
5. Fill in the contract award date and ensure it is formatted correctly.
6. Indicate the month and year when this payment was earned.
7. Input the gross dollar amount due to either contractor or subcontractor.
8. Calculate 1% of the gross amount entered and record it in line 7.
9. Subtract line 7 from line 6 to find the net amount paid; enter this value on line 8.
10. Select the appropriate box for your return type and provide the payment date.
11. Describe the work performed under this contract clearly.
12. Specify the location in Montana where work was conducted.
13. Identify who is submitting this return by selecting from Government Entity, Prime Contractor, or Subcontractor. Sign and provide contact information as required.
