The 1099SA would be filed by your HSA provider if you took any money out of your HSA. The 5498SA would be filed by someone who made contributions to your HSA on your behalf. You would not file these forms, you would receive them if they were applicable.
Participants of an HSA, Archer MSA or MA MSA do not have to file Form 5498-SA with their individual income tax return. However, if distributions from any of these accounts were taxable, they should be noted on Schedule 1 of Form 1040, Part I, line 8e.
You must report any nonqualifying purchases made using funds from your HSA. If you file your return with 1040.com, you simply report these purchases on the 1040.com Health Savings Account screen. Not claiming the nonqualifying expenses may lead to an audit, and youll be subject to penalties and fines.
