Purpose of form. Use Form 4137 only to figure the social security and Medicare tax owed on tips you didnt report to your employer, including any allocated tips shown on your Form(s) W-2 that you must report as income. You must also report the income on Form 1040, 1040-SR, or 1040-NR, line 1c.
The IRS notes that if you fail to report your tips, you could face a penalty equal to 50% of the Social Security and Medicare taxes due on those tips. With that being said, dont hide cash in your pockets. Most tips are given in the form of cash.
DA 4137, also known as the Record of Survey for Army Property, is a document used by the U.S. Army to report the survey of property that has been lost, damaged, or destroyed.
Non-cash tips are those received in any medium other than cash, such as tickets, passes or other goods a customer gives the employee. To determine whether a payment is a tip, the IRS uses a four-factor standard in which all four factors must apply: The customer makes the payment free from compulsion.
(a) Tip offset is the amount of money by which an employer, in meeting the legal minimum wage standard, may reduce a tipped employees wage in consideration of the receipt of tips.

Use Form 8919 to figure and report your share of the uncollected social security and Medicare taxes due on your compensation if you were an employee but were treated as an independent contractor by your employer. By filing this form, your social security earnings will be credited to your social security record.
Your payments of these taxes contribute to your coverage under the U.S. Social Security system. Your employer deducts these taxes from each wage payment. Your employer must deduct these taxes even if you do not expect to qualify for Social Security or Medicare benefits.
