2. Begin by filling out your personal details. Enter your title, family name, given names, address, suburb, state, telephone number, date of birth, tax file number, postcode, and member number. Ensure all mandatory fields marked with an asterisk are completed.
3. In Section 1, choose whether you want to withdraw the full benefit or a partial amount. If opting for a partial withdrawal, specify the amount. Confirm your claim for preserved amounts by ticking the appropriate option based on your employment status.
4. For payment via Electronic Funds Transfer (EFT), provide your bank account details including the financial institution name, BSB number, account number, and account name. Attach proof of your bank account if required.
5. In Section 2, indicate if you wish to transfer all or part of your benefit to another fund and fill in the necessary details about that fund.
6. Complete Section 3 for proof of identification by either providing electronic proof or attaching certified copies of ID documents as specified.
7. Finally, review the checklist to ensure all sections are completed and sign and date the form before submitting it to VISSF.
