2021 schedule a form-2026

1. Click ‘Get Form’ to open the 2021 Schedule A in the editor.
2. Begin by entering your name(s) and Social Security Number at the top of the form.
3. In the 'Medical and Dental Expenses' section, input your total medical expenses on line 1. Then, calculate 7.5% of your modified federal income (line 2) and subtract this from line 1 to complete line 3.
4. For 'Taxes You Paid', check one box for state/local taxes on line 4, then enter amounts for real estate taxes (line 5), personal property taxes (line 6), and any other taxes (line 7). Sum these amounts on line 8.
5. In the 'Interest You Paid' section, fill in mortgage interest details across lines 9a to 12, then sum them on line 13.
6. Complete the 'Gifts to Charity' section by entering cash/check contributions on line 14 and other contributions on lines 15 and 16. Add these for line 17.
7. For 'Casualty/Theft Loss', enter any applicable losses on line 18.
8. List any other itemized deductions in the final sections before calculating total deductions on line 21.
