  6. Prepare your payment by making a check or money order payable to the Kentucky State Treasurer. Do not send cash.
  7. If your entity's name and address are not printed on the check or money order, write them on it along with the FEIN and NRWH account number.
  8. Cut the payment voucher at the dotted line and place both the check or money order and voucher in an envelope without staples or clips.
  9. Mail everything to Kentucky Department of Revenue, Frankfort, Kentucky 40619-0006.
