1. Click ‘Get Form’ to open the 2021 CRP form in the editor.
2. Begin by filling out the Renter/Unit Information section. Enter your first name, last name, rental unit address, city, state, and ZIP code. Make sure to include your Electronic Certificate Number (ECN) and the dates you rented the unit.
3. In the Property Information section, indicate if the property is an Adult Foster Care, Assisted Living, or any other specified type by placing an X in the appropriate box. Also, provide the Property ID or Parcel Number and number of units on this property.
4. For Rent Details, answer whether any rent was paid by Medical Assistance and if you received Minnesota Housing Support. If applicable, enter the amounts in their respective fields.
5. Complete the Total Rent section by entering your share of rent paid and any caretaker rent reduction. Add these amounts to find your total rent.
6. Fill out the Property Owner information including their name and address. Finally, sign and date where indicated to declare that all information is correct.
