1. Click ‘Get Form’ to open the Special Immigrant Visa Biodata Form in the editor.
2. Begin with Section A, where you will enter your NVC Case Number and Assigned Post information. This section is pre-filled by the National Visa Center (NVC).
3. In Section B, provide details about the Principal Applicant. Fill in your name as it appears on your passport, passport number, sex, marital status, date of birth, place of birth, nationality, ethnicity, religion, occupation/skill, phone number(s), email address, education level/field of study, and English speaking ability.
4. Continue to list any children in Section D. For each child, include their name as it appears on their passport, passport number, IV case number, sex, marital status, date of birth, place of birth, nationality, ethnicity, religion, occupation/skill, phone number(s), email address and language abilities.
5. Complete Sections E and F by providing information about any immediate family members processing their own visas and any family or friends residing in the U.S.
6. Review all entries for accuracy before submitting the completed form via email as a scanned attachment.
