The Drug Enforcement Administration (DEA), Office of Diversion Control, will accept requests from distributors that require a large volume of Order Forms (DEA Form 222) with the pin feed tracking left on the form. Order Forms are used for the distribution of a Schedule I or II controlled substance.
An export declaration is mandatory for you to carry out your export. Without a correctly completed export declaration, your shipment may be delayed or even stopped at customs. It can lead to fines and other penalties from the customs authorities. In the worst case, you may be banned from exporting goods in the future.
The rule becomes effective August 28, 2023. The rule revises DEA regulations to provide practitioners with the option of writing prescriptions for controlled substances electronically. The regulations also permit pharmacies to receive, dispense, and archive these electronic prescriptions.
DEA Form 41 is used to request permission from the DEA to destroy controlled substances. The only time University researchers use Form 41 is after controlled substances have been lost due to accidentally spilling or breaking a container.
