2. Enter your Kentucky Nonresident Income Tax Withholding (NRWH) Account Number issued by the Department of Revenue. If you do not have this number, contact Registration at 502–564–3306.
3. Input the month and year of your entity's taxable year ending date in a four-digit format (MMYY). For example, for December 2019, enter '1219'.
4. Provide your Federal Employer Identification Number (FEIN) accurately.
5. Fill in the entity's name, address, contact name, and telephone number. Ensure no punctuation marks are used in the entity's name.
6. Prepare your payment by making a check or money order payable to the Kentucky State Treasurer. Do not send cash.
7. If your entity's name and address are not printed on the check or money order, write them on it along with the FEIN and NRWH account number.
8. Cut the payment voucher at the dotted line and place both it and your payment in an envelope without staples or clips before mailing it to the Kentucky Department of Revenue.
