6. Next, move to the Employee/Payee information section. Enter their first name, last name, home address, city or town, state, ZIP code, and Social Security Number (SSN). If they do not have an SSN or declined to provide it, write 'None' or 'Declined' respectively.
7. The employee/payee must then sign and date their statement confirming they declined to provide an identifying number.
