  6. For each county or municipality where deliveries occur, enter net taxable amounts in Column A and calculate local taxes for Column B based on applicable rates.
  7. Finally, total all net taxable sales and local taxes due on page 7. Make sure to attach all pages of Form ST-389 to your state tax return.
