Arizona Forms 5000 are used to claim Arizona TPT (sales tax) exemptions from vendors. Arizona Forms 5000A are used to claim Arizona TPT (sales tax) exemptions from vendors when making purchases for resale where tax will be collected on the retail sale to the end user.
On May 24, 2022, the City of Globe passed and adopted Ordinance No. 880, which increased the transaction privilege tax rate from two and three-tenths percent (2.3%) to three and three-tenths percent (3.30%). This change has an effective date of August 1, 2022.
The form is an agreement between the seller and the purchaser. Do not send any copies of this form to the Arizona Department of Revenue; the purchaser and seller keeps the form for records. Form 5011 is a questionnaire for the seller to determine if the purchaser qualifies as an out-of-state resident.
Taxpayers also save money through e-filing by becoming eligible to receive an enhanced accounting credit. Taxpayers filing TPT electronically during a calendar year are able to claim an accounting credit of 1.2 percent of the state tax due, with the total calendar year credit limit of $12,000.
