Click ‘Get Form’ to open the Counseling Form in the editor.

Indicate the Type of Occurrence by selecting from Policy/Rules, Performance, Safety, Attendance, Tardiness, Conduct, or Other.
In the 'Describe Issue/Conduct to be Addressed' section, provide a detailed explanation of the issue at hand.
Outline the 'Expected Conduct' that needs to be achieved moving forward.
Encourage employee feedback by having them fill out their comments in the 'Employee Comments' section.
Finally, ensure all parties sign and date where indicated: Employee Signature & Printed Name, Management Signature & Printed Name, and Witness Signature & Printed Name.
