In Section 1, report the number of full-time and part-time employees for each month of the quarter, ensuring you include those who received pay during the relevant pay period.
Move to Section 2. List all employees who had wages during this quarter, entering their Social Security Number, last name, first name, and gross wages paid this quarter. Use 'F' for family-owned businesses and 'X' to delete any employee no longer working.
For contributing employers, complete Section 3 by calculating excess wages and taxable wages based on reported gross wages. Ensure you apply the correct UI tax rate.
Finally, in Section 4, certify that the information is correct by signing and dating the form. Include your contact phone number before submitting.
