How the continuity log works is, for each scene you want to make a note of the scene number and take the camera file numbers and audio file numbers. Most importantly you want to get the notes for the scene. This information is then passed to the editor and the end of the shoot.
What should be included in a script supervisors report?
The report will also include a breakdown of the script pages and scenes. The script supervisor logs how long each take is from action to cut. Most production reports will also contain information from the previous day as well as what still needs to be done.

A script supervisor (also called continuity supervisor or script) is a member of a film crew who oversees the continuity of the motion picture including dialogue and action during a scene.
Maintain continuity of action, e.g. if the subject carries a briefcase in his left hand in one shot, it must stay in the left hand for the subsequent shots.
