  6. Indicate your status by checking the appropriate box—whether you are a CP, NCP, attorney for either party, or an authorized representative. If applicable, include your Oklahoma Bar Association number.
  7. Select the purpose of your request by checking one of the options: support, visitation, or custody.
  8. Type or print your name, then sign and date the form. If you are a CP or NCP, provide your Social Security number again.
  9. Complete your mailing address including street address or P.O. box with city, state, and zip code.
  10. Enter your home and work telephone numbers along with area codes. Optionally include your email address.
  11. Make a copy of the completed form for your records before sending it to OCSS at the provided address.
