Fill in your full name and position within the company, such as Proprietor or Director, to clarify who is submitting the changes.
Indicate what changes you are reporting by ticking the relevant boxes. For example, if you have changed your business contact details, complete Section 1.
In Section 1, provide your new address and ensure it is not an accountant’s or PO Box address. Include any new contact numbers and email addresses.
If changing bank details in Section 2, enter the account name and sort code. Ensure these details are accurate for VAT repayments.
For changes in VAT return dates (Section 3), select your preferred end dates for returns and indicate if you wish to apply for monthly returns.
If transferring ownership in Section 4, provide details of the new owner and confirm whether they wish to retain the existing VAT Registration Number.
Finally, sign and date the declaration section before submitting your completed form to HMRC as instructed.
