For an incapacitated child over age 21 to be considered as a dependent, the child must be unmarried and must have been continuously incapable of self-support due to mental or physical incapacity prior to his 21st birthday (or 23rd birthday if the child is a full time student).
PRINCIPAL PURPOSE(S): The information provided on this form will be used to determine the relationship and dependency of an individual on the military member, for entitlement of authorized benefits.
If a change in dependency has occurred, and you wish for your incapacitated dependent to remain eligible for benefits, you may submit a complete dependency redetermination package (DD Form 137-5) to your parent Service personnel and or finance office, or uniformed services DEERS Project Office.
You may add or remove family members to your DEERS record at the nearest military ID card issuing facility. To determine what documentation you will need to take with you, such as a Birth Certificate, Marriage Certificate, or Divorce Decree, please read the DoD List of Acceptable Identity Documents.
To seek dependent status for an adult family member, such as a father, mother, father-in-law, mother- in-law, stepparent, or someone in loco parentis, DFAS will apply a financial test to determine whether the family member receives over half of his/her income from the Servicemember.
