1. Click ‘Get Form’ to open the Allianz Representative/Agent Change form in the editor.
2. Begin by entering the representative’s name and the name of the broker/dealer in the designated fields.
3. Fill in the representative’s business address, including street, city, state, and ZIP code.
4. Provide the business telephone number and fax number in the respective fields.
5. If there are individual changes, specify the contract owner name and contract number. Indicate if you want your new representative to have telephone transfer access by adding your initials.
6. For block changes, check the appropriate box and attach a list of contract owners’ names or contract numbers as needed.
7. Sign and date the form where indicated for individual changes or have the previous branch manager sign for block changes.
8. Once completed, save your document and follow instructions to fax or mail it to Allianz Life Service Center.
