Fill in the Requisition Number, ensuring it appears on all related correspondence. This is crucial as it is not an order but a reference number.
Provide your contact details in the 'Return Quote to' section, including Name, City, State/Province, Company, Country, Zip/Postal Code, Address 1 and Address 2. Make sure these are accurate for effective communication.
Enter your Telephone and Fax No. for any follow-up queries regarding your quote.
Specify the deadline for receiving quotes in the 'Quote to us by' field along with any Requested Terms and Requested F.O.B.
List each item you are requesting a quote for under 'Item', including Qty. Ordered, Description, Unit Count, Unit Price, and Total fields.
Finally, sign and date at the bottom of the form to authorize your request before submitting it.
