Youll use the my NDIS portal or app to make a claim. If you dont have a plan in our new computer system, youll continue to use the myplace portal instead of the my NDIS portal. If you cant make a claim using the portal or the my NDIS app, youll need to use the MyPlan purchase payment request form (DOCX 85KB).

Information for your service providers about what supports they have delivered and what supports you need to reach your goals for the next year.
For children younger than 9, we aim to approve your plan within 90 days after you become a participant. If youre 9 or older, we aim to approve your first plan within 56 days after you become a participant.

Once your current NDIS plan ends, your funds stop. And any unspent funding will NOT roll over into your new plan. After each plan review you will receive a new plan outlining supports and funds for the next period of time.
