Introduction: Problem Oriented Medical Record (POMR) is a medical record approach that provides a quick and structured acquisition of the patients history. It was born in the late sixties. Expecting an ordered, complete and updated medical record were some of the goals.
Source-oriented (SO) charting is a narrative recording by each member (source) of the health care team charts on separate records. SO charting is time-consuming and can lead to fragmented care. Problem-oriented medical record (POMR) charting was introduced by Dr. Lawrence Reed to focus on the clients problem.
A source-oriented record is one in which each healthcare group keeps data on its own separate form. Sections of the record are designated for nurses, physicians, laboratory, x- ray personnel, and so on. Notations are entered chronologically, with the most recent entry being nearest the front of the record. (
In the source-oriented method, the health care providers have separate forms for each other. In the problem-oriented record, there is only one form the health care providers write. Last, the problem list is a combination table of contents and index to the patients condition and progress.
