2. In Item 1, ensure the preprinted name is correct. If not, attach a statement with the correct name and date of amendment.
3. For Item 2, enter the file number issued by the California Secretary of State if it is blank.
4. In Item 3, if applicable, enter the state where your limited liability company is organized.
5. Complete Item 4 with the full street address, city, and zip code of your principal executive office. Avoid using P.O. Boxes.
6. For Item 5, provide the complete address for your California office where records are maintained.
7. In Items 6 through 9, list names and addresses of any managers or members as required. Attach additional pages if necessary.
8. Item 10 requires you to enter the name of your agent for service of process in California. Ensure they agree to this role.
9. If an individual is designated as agent, complete Item 11 with their California address; leave it blank if a corporation is designated.
10. Describe your business type in Item 12 and complete Item 13 with your name, title, and date when finished.
