Customer set up form template 2026

1. Click ‘Get Form’ to open the customer set up form in our platform's editor.
2. Begin by filling out the 'General Information' section. Select whether your company is 'Taxable' or 'Exempt' and provide the necessary details such as Company Name, Contact Person, Title, Address, City, State, Zip, Phone, and Email.
3. If you selected 'Exempt', ensure to fax an Exempt Certificate as required. For payment terms, indicate your preferred credit terms under 'Requested Credit Terms'.
4. In the 'Billing Information' section, if it’s the same as above, check the corresponding box. Otherwise, fill in the Billing Name and Address details.
5. Provide your Credit Card information if applicable. Ensure to include Credit Card Number, Expiration Date, and Security Code.
6. For 'Shipping Information', repeat the process or check if it’s the same as billing. Fill in Shipping Name and Address accordingly.
7. Finally, type your name in the signature box to confirm approval for credit card usage before clicking ‘Submit’.
