Begin by filling out the Background Information section. Enter the individual's name, unique ID number, case number, birthdate, age, and address.
Proceed to Pages 2 and 3. For each Domain grid, read down each column until you find the level that best describes the individual’s current needs. Mark an 'X' in the corresponding box.
After completing all grids, circle the marked levels in the summary chart below and connect them with lines for clarity.
Record the highest score for each domain in their respective boxes and write this highest score in the 'Overall Level of Eligible Support' box.
Finally, move on to Page 4 for the NC-SNAP Support Summary where you will summarize identified support needs and preferences.
