  5. Indicate whether the patient can perform all activities of daily living (ADL) independently by marking 'Y' or 'N', and circle any ADLs requiring assistance.
  6. Complete the Medication Review List by indicating if the member is on medication with an 'X' for YES (Y) or NO (N), along with practitioner details.
  7. Fill out the Pain Assessment section by marking under pain management plan and treatment. Rate level of pain from 1 to 5 and add any comments or additional information.
