1. Click ‘Get Form’ to open the health care assessment in the editor.
2. Begin by entering the date of patient assessment, patient name, and date of birth in the designated fields.
3. Fill in the member ID and physician name to ensure proper identification.
4. Record the patient's weight, height, and calculate BMI value as needed.
5. In the Functional Assessment section, circle the appropriate options for overall assessment and provide additional details regarding cognitive status, ambulation status, speech, hearing, vision, touch, and smell/taste.
6. Indicate whether the patient can perform activities of daily living independently by marking 'Y' or 'N' and circling any assistance required.
7. Complete the Medication Review List by indicating if the member is on medication with an 'X' for YES (Y) or NO (N), along with practitioner details.
8. Finish with the Pain Assessment section by documenting pain management plans and levels of pain experienced by the patient.
