2. Begin by filling in your District and Unit Number at the top of the form. Ensure all information is accurate.
3. Complete the personal details section, including First Name, Middle Initial, Last Name, Address, City, State, Zip Code, Birth Date, Years as Adult, Home Phone, and Email Address. Print clearly for legibility.
4. In the Training Completed section, list any relevant training you have undergone. This helps showcase your qualifications.
5. Detail your Positions Held and Scout as Youth / Rank to provide context on your experience within Scouting.
6. Fill out Community Activities and Employment / Trade sections to highlight your contributions outside of Scouting.
7. Address the Camping Requirement by listing dates and locations of long-term camping experiences that fulfill the criteria outlined in the form.
8. Nominate yourself or another adult leader by signing and dating the form at the bottom. Ensure both Unit Leader and Committee Chairman signatures are included.
