File Form 1099-R for each person to whom you have made a designated distribution or are treated as having made a distribution of $10 or more from: Profit-sharing or retirement plans. Any individual retirement arrangements (IRAs).
Say your IRA was worth $500,000 at the end of 2021 and you're turning 72 in 2022. The IRS distribution period for 72-year-olds is 27.4 years. So, if you divide $500,000 by 27.4 years, you get $18,248. That's what your RMD for 2022 would be.
To correctly report the distribution and rollover on Form 1040, the retiree must include the full amount of the distributions on line 4a. If the entire amount was rolled over, or returned, to the IRA, put zero on line 4b as the taxable amount.
Any RMDs for the year 2022 will start using the new table and distribution period factors. For all subsequent years after your reach your RMD age, including the year in which you were paid the first RMD by April 1, you must take the RMD by December 31 of that year.
The SECURE Act 2.0 increases this threshold to those age 73 on Jan. 1, 2022, to those age 74 on Jan. 1, 2030, and to those age 75 on Jan. 1, 2033.

Form 5498 reports IRA contributions, rollovers, Roth IRA conversions, and required minimum distributions (RMDs) to the IRS. Your IRA trustee or custodian is the one responsible for mailing Form 5498 to the IRS, along with a copy to you. You don't have to do anything with the form itself.
You will receive a Form 1099-R when you make a withdrawal from a IRA, 401(k) or other retirement account. This form includes information such as: the amount you withdrew, how much is taxable (if that was determined), any taxes that were withheld, and a code that shows what type of distribution it was.
Regardless of your age, you will need to file a Form 1040 and show the amount of the IRA withdrawal. Since you took the withdrawal before you reached age 59 1/2, unless you met one of the exceptions, you will need to pay an additional 10% tax on early distributions on your Form 1040.
How are RMDs taxed? The account owner is taxed at his or her income tax rate on the amount of the withdrawn RMD. However, to the extent the RMD is a return of basis or is a qualified distribution from a Roth IRA, it is tax free.
Any RMDs for the year 2022 will start using the new table and distribution period factors. For all subsequent years after your reach your RMD age, including the year in which you were paid the first RMD by April 1, you must take the RMD by December 31 of that year.
