How to use or fill out L&I Form guidelines.doc with our platform

- Locate the section for the interpreter’s provider number. This should be your individual provider number under our company. If you are unsure, please consult your supervisor or ask for assistance.
- If you wish to be reimbursed for mileage, make sure to fill out all address fields completely. This ensures that reimbursement can be processed without delays.
- For group services, remember that each client requires a separate form. Do not combine multiple clients on one form to avoid complications.
- The bottom section of the form is strictly for provider completion, not interpreters. Fill this out accurately to ensure proper billing.
- When billing your time, do so in minutes as payment will be calculated accordingly. Note that no payment will be made for interpreting time in case of a no-show; however, mileage will still be reimbursed.
