5. For group services, remember that each client requires a separate form. Do not combine multiple clients on one form to avoid complications.
6. The bottom section of the form is strictly for provider completion, not interpreters. Fill this out accurately to ensure proper billing.
7. When billing your time, do so in minutes as payment will be calculated accordingly. Note that no payment will be made for interpreting time in case of a no-show; however, mileage will still be reimbursed.
