To comply with immunization requirements, you must complete the medical clearances section in your Patient Portal. Log into the portal, look for the Medical Clearances tab on the left and update any items marked Non Compliant by submitting documentation.
Students in public institutions of higher education must be immunized against Tdap, MMR, meningococcal disease and hepatitis B. A religious exemption is permitted. Public and private colleges that offer on-campus or group housing must provide information on meningococcal disease to each first-time student.
Sign into your account or the app. Under Health History Dashboard, select Health Summary. From there, select Immunizations. MyChart can even pull in records from vaccines you received at other locations, such as a pharmacy.
On the portal home page, click on Immunization/TB Upload on the left menu bar. c. On the Immunization/TB Upload page, click on the Add Immunization Record button and navigate to your scanned completed document (e.g., Immunization records, TB Health Assessment Form, etc).
Students will be charged a $15 co-pay for PUSH ancillary services such as physical therapy and X-ray. Students must pay a $200 deductible for in-network preferred providers and a $400 deductible for out-of-network providers for all services outside of PUSH.

Follow this process: Use your NetID and password to log into the patient portal. The Student Immunization Form is found in the patient portal under FORMS. Complete the form by entering the dates of immunizations on your immunization record. Upload a copy of your immunization record where indicated.
Ask your doctor, pharmacist or other vaccine provider for an immunization record form or download and use this form [4 pages]. Bring this record with you to health visits, and ask your vaccine provider to sign and date the form for each vaccine you receive.
