Contact Form Navigate to facebook.com/contact.form and sign into your account. Click Go to App and then click Allow. Click the Select a page drop-down menu and select the page on which you want to add a contact form. Enter your name, email address and website address in the appropriate text fields.
Business Suite Log into your Facebook account and go to your Page. Go to Publishing Tools. From there, you will need to go to the All tools section. Click on Instant Forms. This will show you all the forms on the page and the leads collected from each form.
