hey everyone its Liz with Camp woody today I will be showing you where to find the camper health history form and how to upload it so once you get into your account you will click the three lines in the top left corner you will click document Center here are all of the documents that you might need but we just want this camper health history form we will download document you will indicate which camper it is for and then itll bring you to your camper hell history form so the only thing that it fills out for you is this bar everything else is blank that youll fill out additional information about the camper if they have any dietary restrictions their immunization history if theyve ever been hospitalized just general information so you will print this out you will then fill it out and scan it back in you can either mail this to us or I can show you how to upload it yourself to your account so we will go back to document Center and we will select who it is for and then you will update
